Functional Medicine Forms
Please fill out the forms below and fax or email them to the office at least 3 days prior to your first appointment along with any old lab work you may have. In order to schedule your Functional Medicine appointment we must have your information in hand. Your case is reviewed before the doctor meet's with you which allows the doctor to have a better understanding of your current condition prior to your arrival.
If you need to reschedule your appointment, we must have at least 48 hours notice as a large time slot is booked out as well as prep time for your initial Functional Medicine visit.
